- You will not receive a refund, if you fail to properly prepare for your testing appointment. Please review test preparation information here: TEST PREPERATION.
- If you purchase a test package. All services must be completed in one testing appointment. Unless there is a valid reason all test can’t be completed, you will not receive a refund for the test service you didn’t utilize.
- If you are an individual who selected any 2 x package, book your first day & email us at email@example.com to book your follow-up appointment. You will not receive a refund if you change your mind about your follow-up appointment or forget to schedule your follow-up appointment.
- If you are facility/ trainer who selected any 2 x package, book your first day & email us at firstname.lastname@example.org to book your follow-up appointment. You will not receive a refund if you change your mind about your follow-up appointment or forget to schedule your follow-up appointment.
- All before and after packages (2 x packages) must be used within 90 days or you lose credits! NO ROLLOVERS!
- Your testing appointment can be gifted if for some reason you can’t make it to your appointment time. You must email email@example.com with (Name of Person & Phone) and CC: the person who you gift your appointment on the email, within 48 hours of appointment time.
- Effective 3/18/2019traveling to you incurs a $50 charge. If you oversleep or forget, you will forfeit what you paid for your service and travel fee.
- You can modify or cancel up until 24 hours before appointment and still keep service credit. If you cancel and it less than 24 hours before you forfeit 100% your payment.
- We don’t charge any type of cancel or reschedule fee. See 7 & 8.
Unless otherwise provided by law, you understand and acknowledge all sales are final, and no refunds outside of the refund policy listed will be provided at any time.
